Benefits at David Yurman are thoughtfully crafted and expertly curated to provide you with a comprehensive and competitive total rewards package. Enrollment Specifications

ELIGIBILITY

For you: You are eligible for health and welfare benefits if you are a full-time employee regularly scheduled to work at least 30 hours per week (or the minimum number of hours to comply with state mandates). Exempt Employees are salaried and not eligible for overtime. Non-Exempt Employees are hourly and eligible for overtime. Benefits become effective the first of the month following your date of hire for exempt employees or the first of the month following 60 days from your date of hire for non-exempt employees.

For your dependents: Your dependents become eligible for coverage on the same date you do. Eligible dependents are your:

  • Legal spouse
  • Domestic partner
  • Children up to age 26, including stepchildren, foster children and adopted children (for medical, dental and vision)
  • Disabled child of any age (with documentation) who is dependent on you for support due to a mental or physical handicap that occurs before reaching age 26

ENROLLMENT

If you are a new hire, you will need to submit your elections in Workday within 30 days of your Date of Hire.

For 401K, Commuter Benefits and Pet Insurance, you enroll directly through the carrier’s website. There is no waiting period for 401(k) or Commuter Benefits. You may enroll and make changes to your elections at anytime.

Important: If you do not enroll when first eligible, during the annual open enrollment period or within 30 days of a qualifying life event, you must wait for the next annual open enrollment period.

MID-YEAR CHANGES

The IRS requires that benefit elections paid for on a before-tax (pre-tax) basis remain in effect for the full plan year. However, the IRS permits mid-year changes within 30 days of a qualifying life event.

Examples of qualifying life events are:

  • Your marriage, divorce, legal separation or annulment
  • The birth of your baby, or adoption or placement of a child with you for adoption, or another change in the number of your dependents
  • The death of a dependent
  • Your dependent’s eligibility or ineligibility for coverage (for example, he or she reaches the plan’s eligibility age limit)
  • A change in coverage of your spouse or your dependent under another plan
  • Your qualification for a special enrollment under the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
  • A court order received by the plan, such as a Qualified Medical Child Support Order (QMCSO)
  • You, your spouse or your dependent’s qualification for Medicare or Medicaid

Care Line is an NFP-sponsored program that is staffed by dedicated professionals to help you understand the benefit options made available to you. Whether you have concerns about a recent claim or bill, finding an in-network doctor or just some guidance on which medical plan is right for you and your family, the Care Line can help educate and advocate. The Care Line is available to all employees and children over 18 who are enrolled on the benefit plan.
